At Unity CareLink, our mission is to make disability support less fragmented and more human. We bring families, DSPs, agencies, and community programs into one shared space so the person at the center of care is no longer hidden behind paperwork, separate apps, and disconnected conversations.

We design technology that supports real relationships; clear communication, shared routines, and strengths based profiles that show who a person is not only their diagnosis.

Our vision is a network where every individual with different abilities is visible, valued, and surrounded by a coordinated circle of support.

By combining lived experience in the field with thoughtful, evidence informed design, Unity CareLink aims to reduce everyday chaos, support caregiver wellbeing, and create more consistent, dignified care for the people and families who deserve it most.

Our Story

Built From Real-World Needs

Unity CareLink was built for one reason: care coordination is harder than it needs to be.

Families, DSPs, agencies, and programs often work with the same person yet updates get scattered across texts, notebooks, portals, and shift changes. Over time, that division creates stress, missed details, and a person who feels “managed” instead of truly supported.

We created Unity CareLink to bring the whole support circle into one place through messages, schedules, notes, and strengths based profiles so care becomes clearer, more consistent, and more human.

Our goal is simple; reduce chaos, increase visibility, and help people with disabilities be seen for who they are.
